Job summary

CommonSpirit Health is seeking a Director of Care Management to provide overall coordination across the acute care continuum. You will establish a comprehensive program that includes case management, utilization review, care coordination, discharge planning, and social work.

You will supervise all Case Management Services (acute, rehab, outpatient), manage staff education and performance, and oversee the department budget while serving as a clinical resource and liaison with external agencies

Qualifications

• Requires RN license or LMSW/LCSW license.
• 5 years of nursing or social work experience, with 3+ years in hospital case management.
• Prior supervisory/management experience in Case Management.
• Experience with discharge planning and clinical utilization review.

Responsibilities

• Provide overall coordination for care management across acute care continuum.
• Supervise Case Management Services (acute, rehab, outpatient) including staff development.
• Manage departmental budget and oversee productivity.
• Coordinate UM committees and reports and act as liaison with external agencies.
