Job summary

Providence is seeking a Director, Care Management to oversee care transitions, throughput and continuum utilization across acute care ministries. The DCM leads case management, utilization management, discharge planning and psychosocial services to ensure safe, patient-centered care.

The role entails strategic planning, team leadership, budget oversight, and performance reporting to meet quality, outcomes and Triple Aim goals, with strong collaboration across medical staff and community partners.

Qualifications

• Bachelor’s Degree in Nursing (RN) and California RN license required.
• Master’s Degree in Health-related field preferred; MSW if non-nursing.
• 2 years of care management experience in an acute care setting.
• Knowledge of principles of acute case management and evidence-based practice.
• Proficiency with Microsoft Office products (Word, Excel, PowerPoint, Outlook).

Responsibilities

• Oversee care transitions, throughput, and continuum utilization across acute care ministries.
• Directly supervise staff and foster professional development.
• Develop and execute strategic work plans with quality, clinical and financial goals.
• Budget for areas of responsibility and monitor variances against plan.
• Collaborate with medical staff and community care partners to support transitions and patient experience.
• Ensure regulatory compliance and advancement toward the Triple Aim.
