• Minimum 3 years of leadership experience.

Responsibilities

• Oversee care management and social work with 24-hour accountability within budget and regulatory constraints.
• Ensure compliance with regulatory bodies and organizational missions.
• Provide leadership to the care coordination team and collaborate with Hampden and Holy Spirit centers.
