Job summary

Qualifications

• Completion of Vocational Nursing Program is required.
• Minimum 2 years ambulatory clinic experience is required.
• 1 year supportive palliative care experience is preferred.
• 1 year acute care experience is preferred.
• Licensed Vocational Nurse certification is required upon hire.

Responsibilities

• Provide clinical care coordination and administrative support to interdisciplinary care teams under RN/provider direction.
• Serve as a liaison between patients, caregivers, providers, and external partners to facilitate ongoing clinical care coordination.
• Assist with medication management by identifying barriers and coordinating refills and prior authorizations.
• Reinforce education and escalate medication-related concerns to RN/provider as appropriate.
• Follow up on patients in acute care settings and refer for additional services when needed.
• Support referral management by pre-screening referrals and initiating service requests as directed.
• Assist patients and families with navigating care processes and plan of care and follow up steps.
• Support transitions of care and follow‑up coordination in acute or post‑acute settings.
• Document patient contacts and nursing activities in the EHR.
• Promote patient knowledge of conditions and self-management.
• Participate in daily meetings and education sessions to support care and quality initiatives.
• May support home nursing visits and residential visits as directed.
