Job summary

Virtua Health is seeking an experienced RN or Masters prepared Social Worker to coordinate patient care across a multidisciplinary team. The role emphasizes collaboration with physicians, social work, and case management to ensure quality, cost-effective care.

On-site in Lourdes Health System locations, strong documentation and compliance are required. Minimum 3 years RN or Social Work experience plus 1 year in case management or homecare coordination in acute or behavioral health settings.

Qualifications

• Minimum 3 years clinical nursing (RN) experience or Social Work.
• Minimum of 1 year Case Management or other interdisciplinary experience including Homecare Coordination in an Acute Care setting or behavioral health in an inpatient outpatient or integrated behavioral health setting; understanding of Medicare, Medicaid, managed care and discharge planning.

Responsibilities

• Care Coordination - assess and plan effectively to meet patient needs, monitor length of stay and optimize resources.
• Documentation - complete documentation of plans of care in EMR and case management systems.
• Engage physicians and multidisciplinary teams to ensure seamless continuity of care and effective communication.
• Identify factors impeding patient progression, resolve and escalate as appropriate, refer to Social Work as needed.
• Participates in organizational improvement activities including patient satisfaction and Six Sigma teams.
