Dignity Health in Chandler, AZ, seeks a Care Coordination Manager to oversee daily case management operations, discharge planning, utilization review, and social work interventions, collaborating with physicians and administrators to deliver high-quality patient care.

You will lead staffing, performance improvement, reporting, and ensure regulatory compliance across the department, driving timely care coordination and resource utilization in a 443-bed acute care setting.

Qualifications

• Bachelor's Degree in Nursing or Master's in Social Work required or equivalent experience.
• Three years of clinical care coordination or acute nursing leadership experience.
• Arizona RN or LMSW licensure and ACM/CCM certification within 24 months.

Responsibilities

• Provide operational leadership for Care Coordination through staffing, supervision, and performance management.
• Coordinate programs and workflows to support patient progression and quality outcomes.
• Oversee daily operations, scheduling, workload balancing, and resource allocation.
• Ensure timely patient assessments, discharge planning, and care transition activities per standards.
• Monitor metrics, implement improvements, and escalate issues to the Director.
