Responsibilities

• Review resumes and conduct interviews.
• Train, coach, and provide feedback to team.
• Onboard new staff with Health Homes Lead.
• Provide updates to Program Director and Lead.
• Supervise care coordination staff and perform evaluations.
• Facilitate weekly/monthly team meetings.
• Manage referrals and monitor project goals.
• Assist in data reporting and audits.
• Collaborate with Health Homes Leads on service delivery.
