Job summary

Columbia River Mental Health Services is seeking a Health Homes Assistant Manager for Care Coordination in Vancouver, WA. The role emphasizes supervision of care coordinators, staff development, and collaboration with Health Homes leads to ensure high-quality, timely services for clients in the community.

Qualifications

• Approximately 3+ years of experience in the social service field with mental illness, medically fragile and/or chemical dependency.
• At least 1+ years of experience in a supervisory or lead role in a social service field is required.

Responsibilities

• Participation in reviewing resumes, conducting interviews.
• Providing training, coaching, and feedback to the team.
• Onboarding new staff with guidance from Health Homes Leads.
• Provide regular updates to Program Director and Program Lead on training and coaching.
• Supervise care coordination staff and conduct staff evaluations.
• Facilitate weekly/monthly team meetings.
• Manage referrals from Leads and monitor project goals and outcomes.
• Collect data for reporting and billing and participate in state audits as needed.
• Engage with community partners for outreach and coordination of care.
