The University of Maryland Medical System is seeking a Care Management Technician to support discharge planning and care coordination under supervision of an RN Case Manager or Social Worker.

You will create discharge packets, arrange transportation, deliver DME, and communicate with patients and families using email, phone, and EMR systems. CNA preferred.

This role requires strong communication, organizational skills, and the ability to work under pressure in a hospital setting.

Qualifications

  • Customer service experience is preferred.
  • Experience in hospital or health care is advantageous.

The care management technician assists by supporting the core functions of the department and discharge planning.

Under supervision of an RN Case Manager, RN Utilization Manager or Social worker, the care management technician provides a variety of services to the department that requires thorough knowledge of departmental practices and procedures. The care management technician assists by supporting the core functions of the department and discharge planning.

Principal Responsibilities And Tasks

The following statements are intended to describe the general nature and level of work being performed by people assigned to this classification. They are not to be construed as an exhaustive list of all job duties performed by personnel so classified.

  • Creates discharge packets for patients going to skilled nursing facilities. Creating the discharge packet requires working with several disciplines to gather needed documentation such as the discharge summary, MOLST, medication administration record, etc.
  • Delivers durable medical equipment (DME) to the patient’s room for discharge and completing any associated insurance or charity paperwork.
  • Confirms home care start of care.
  • Delivers clothes suitable to the weather to the patient’s room for discharge.
  • Completes patient screenings.
  • Makes transportation arrangements for patient’s discharge including speaking with family to arrange pick up, providing bus tokens, making arrangements with LYFT
  • Delivers notices to the patient including, but not limited to, the Medicare Outpatient Observation Notification and the Important Message from Medicare.
  • Assists with communication with patients and families, payers, customers, and department staff through email, encrypted text, phone, fax, virtual phone conferences, and filing in the share drive.
  • Documents in medical record.
  • Provides support to special projects and initiatives.
Knowledge, Skills And Abilities
  • Highly effective verbal, written and interpersonal skills to communicate effectively with patients, families, and interdisciplinary hospital staff is required
  • Demonstrated customer service skills required
  • Experience in hospital or health care is preferred
  • Experience with medical terminology is preferred
  • Demonstrated ability to maintain organizational system is required
  • Demonstrated ability to work under pressure is required
  • Demonstrated ability to work with basic computer programs (email, word, electronic medical record) is required
Education And Experience
  • High School diploma or GED is required
  • CNA certification preferred
