Job summary

The University of Maryland Medical System is seeking a Case Manager to provide care coordination and discharge planning for an assigned patient caseload under general supervision. You will assess patient needs, coordinate plans of care, and monitor outcomes across the continuum of care.

Responsibilities include collaborating with multidisciplinary teams, leading discharge planning, ensuring timely authorizations, and supporting performance improvement initiatives while maintaining patient safety

Qualifications

• MD RN licensure or equivalent required.
• Experience with discharge planning and case management.
• Strong written and verbal communication skills.

Responsibilities

• Assess patients and integrate psychosocial assessment within the multidisciplinary team.
• Develop and coordinate Discharge Plan A and Plan B with documentation.
• Collaborate with physicians, nursing, social work, and other departments to eliminate barriers to care.
• Facilitate timely transition plans and reduce readmissions.
• Lead care management rounds and integrate patient stories into the plan of care.
• Ensure timely authorizations and support clinical performance improvement.
