Hackensack Meridian Health is hiring a Care Coordinator in Care Management to coordinate, communicate, and facilitate the clinical progression of patients’ treatment and discharge plans. You will manage a designated caseload and work with families and the healthcare team to meet goals and arrange the next level of care.

The role requires a BSN or current pursuit, or an MSW, with strong communication and collaboration skills, and familiarity with Epic or similar platforms.

Formación

• BSN or BSN in progress and/or willing to acquire within 3 years of hire; or MSW degree.
• Strong decision-making and problem-solving abilities with leadership potential.
• Excellent verbal, written, and presentation skills.
• Moderate to advanced computer skills and resource knowledge.
• Familiarity with hospital and community resources; utilization management.

Responsabilidades

• Coordinate discharge planning and progression with patients, families, and clinicians.
• Facilitate interfacility transitions and handoffs between acute and post-acute care.
• Document care plans, education, referrals, and authorizations in the medical record.
• Participate in multidisciplinary rounds and quality management activities.
• Identify barriers to care and connect patients with appropriate resources.
