Job summary

Hackensack Meridian Health in New Jersey is seeking a Care Coordinator, Care Management to coordinate, communicate, and facilitate the clinical progression of a patient’s treatment and discharge plan. You will assess, plan, and facilitate with patients, families, and the multidisciplinary team to meet treatment goals, expected length of stay, and arrange for the appropriate next level of care.

The role requires RN or social work licensure, and emphasizes collaboration, education, and utilization

Qualifications

• BSN or BSN in progress and/or willing to acquire within 3 years of hire; or Masters in Social Work.
• Effective decision-making skills and leadership abilities.
• Excellent verbal, written and presentation skills.
• Moderate to expert computer skills.
• Familiar with hospital resources and utilization management.
• Excellent written and verbal communication skills.
• Proficient with Microsoft Office and Google Suite platforms.

Responsibilities

• Assess patients for discharge needs and develop individualized care plans with physician input.
• Coordinate communication among health care team and involved family members.
• Refer patients to community resources and ensure appropriate levels of care.
• Collaborate with multidisciplinary teams to secure timely transitions.
• Develop discharge plans addressing ongoing care needs.
• Document care coordination and regulatory communications in medical records.
• Participate in committees and contribute to quality improvement.
• Identify quality issues for review and support problem resolution.
• Engage in rounds and discuss discharge and length of stay.
• Reassess progress and revise plans as clinical conditions change.
