Job summary

Hackensack Meridian Health is hiring a Care Coordinator, Care Management to manage patient discharge planning and care transitions. You will partner with physicians, nurses, and social workers to develop individualized plans that meet goals and length-of-stay targets.

The role requires collaboration across disciplines, knowledge of CMS guidelines, and strong communication to engage patients and families in decision making.

Qualifications

• RN or social work background with relevant licensure.
• 2:00
• Ability to coordinate discharge planning with a multi-disciplinary team.
• Knowledge of hospital resources and community resources.

Responsibilities

• Coordinate and facilitate discharge planning for patients with the medical team.
• Communicate with patient, families, and providers to implement the care plan.
• Ensure timely transitions between acute and post-acute services.
• Maintain accurate documentation and participate in committees as assigned.
• Educate patients on discharge options and available benefits.
