Hackensack Meridian Health is seeking a Care Coordinator in Care Management to coordinate, communicate, and facilitate the clinical progression of patient treatment and discharge plans. You will manage a patient caseload and collaborate with families and the multidisciplinary team to meet goals and arrange the next level of care.

Ideal candidates hold a NJ-licensed RN or SW credential, possibly a Master's degree, and demonstrate strong decision-making, communication, and computer skills to

Qualifications

• BSN in progress and/or willing to acquire within 3 years of hire; or Master's Degree in Social Work.
• Effective decision-making and problem-solving skills, with leadership ability.
• Excellent verbal, written and presentation skills.
• Proficient with Microsoft Office and Google Suite; strong computer skills.
• Familiar with hospital resources, utilization management.

Responsabilités

• Coordinate, communicate, and facilitate the clinical progression of the patient's treatment and discharge plan.
• Accountable for a designated patient caseload; assesses, plans, and facilitates with patients, families and the multidisciplinary team to meet treatment goals, expected length of stay, and arrange for the appropriate next level of care.
• Oversees interfacility transitions and handoff between acute and post-acute services.
