Job summary

Duke Health in Durham, North Carolina, seeks a Case Manager to coordinate discharge planning and ensure seamless care transitions across the continuum. You will assess patients, develop individualized discharge plans, and collaborate with the healthcare team to achieve timely, efficient outcomes.

Requires BSN or MSW, a minimum of 3 years’ experience, and Case Management Certification within 3 years of hire. BSN candidates must hold an NC RN license.

Qualifications

• BSN or MSW degree is required.
• Minimum of 3 years of relevant case management experience.
• Case Management Certification required within 3 years of hire; NC RN license for BSN-prepared candidates.

Responsibilities

• Assess assigned patients for case management needs, including discharge planning and care transitions.
• Develop, implement, and evaluate individualized discharge plans.
• Coordinate services such as Home Health, DME, transportation, SNF, and rehab referrals.
• Maintain timely and accurate documentation of assessments and actions.
• Provide weekend/holiday/after-hours coverage as assigned.
