Job summary

Duke University Health System is seeking a Case Manager in Durham, NC to oversee assigned caseloads, coordinate discharge plans, and ensure efficient care transitions across the continuum. The role requires strong communication, independent problem solving, and a BSN or MSW with 3+ years of relevant experience.

Relocation assistance and a commitment bonus may be available for eligible candidates. A Case Management Certification must be obtained within 3 years of hire and North Carolina RN

Qualifications

• BSN or MSW required.
• Minimum of 3 years of relevant experience.
• Case Management Certification (ACM, CCM, or ANCC) required within 3 years of hire.
• RN license (North Carolina) or compact license for BSN-prepared candidates.

Responsibilities

• Assess patients for case management needs including discharge planning and care transitions.
• Develop, implement, and evaluate individualized discharge plans.
• Coordinate Home Health, DME, transportation, and SNF referrals.
• Maintain timely documentation and ensure compliance with regulations.
• Collaborate with interdisciplinary teams to coordinate safe transitions of care.
