Job summary

Duke Raleigh Hospital, a campus of Duke University Health System, is seeking an experienced Case Manager to coordinate discharge planning and transitions of care. You will collaborate with the healthcare team, patients, and families to ensure timely, safe outcomes across the continuum of care.

Qualifications include a BSN or MSW, 3+ years of experience, and a Case Management Certification within 3 years of hire; NC RN license required for BSN candidates.

Qualifications

• BSN or MSW required.
• Minimum of 3 years of relevant experience.
• Case Management Certification (ACM, CCM, or ANCC) within 3 years of hire.
• For BSN-prepared candidates: NC RN license (or compact license).

Responsibilities

• Care Coordination & Discharge Planning: assess patient needs and develop discharge plans.
• Collaborate with interdisciplinary teams and care partners to ensure safe transitions.
• Documentation & Compliance: maintain accurate records and ensure regulatory adherence.
• Quality & Professional Development: participate in QAPI activities and ongoing education.
• Provide weekend, holiday, or after-hours coverage as assigned.
