Job summary

Mercy is seeking a Care Management Associate to support the Care Management team with transition planning and length of stay management. You will coordinate information flow, communicate with patients and families, and assist with logistics like prescriptions, transport quotes, and documentation in Epic EMR.

The role emphasizes collaboration with healthcare professionals, adherence to Mercy service standards, and maintaining confidentiality while improving patient experience and care

Qualifications

• 1 year healthcare experience or healthcare student experience preferred
• Excellent interpersonal and organization skills
• Ability to work with and preserve confidential information
• Strong verbal and written communication and facilitation skills
• Strong time management, prioritization, and critical thinking skills
• Knowledge of Epic, preferred

Responsibilities

• Coordinate information and communication for the Care Management team
• Deliver and retrieve prescriptions to Mercy Tower Retail Pharmacy
• Obtain quotes from transport companies for patient transfer
• Deliver patient letters and facility lists to patient/family
• Support care management plans to meet Mercy facility needs and metrics
• Print and distribute IMM letters and document in Epic EMR
• Identify and resolve problems with the care management team
• Maintain knowledge of Mercy policies and regulatory requirements
