Job summary

Mercy Health is seeking a Manager, Care Management to lead day-to-day operations of the Care Management Department across multiple locations. The role combines clinical and financial aspects to optimize outcomes for patients and the organization.

You will collaborate with leadership to standardize processes, oversee utilization management, and guide transitions of care while ensuring compliance with regulatory standards.

Qualifications

• RN license in state of practice or Social Worker license in state of practice (required)
• BLS Basic Life Support – American Heart Association (required)
• 3 years of recent acute hospital care management experience (required)

Responsibilities

• Provides leadership and oversees day-to-day operations of the Care Management Team.
• Advocates and educates on right care, right time, at the right place.
• Utilizes systems to support proactive care management across the continuum.
• Partners with physician leadership to optimize utilization and data analysis.
• Directs daily operations and participates in budget planning and cost reduction efforts.
