Mercy Hospital Springfield is seeking a Care Management Associate to support the Care Management team in transition planning and length of stay management. This full-time role focuses on coordinating information and communications to ensure patients receive the right information at the right time.

Ideal candidates have a High School Diploma with about 1 year of healthcare experience; BLS (CPR) at hire date or within 90 days is required.

Qualifications

• Education: High School Diploma, minimum required.
• Experience preferred: 1 year Healthcare experience, may include Healthcare student experience. Recent hospital experience.

Responsibilities

• Coordinate information and communication for the Care Management team.
• Support transition planning and length of stay management.
• Perform duties and responsibilities in a manner consistent with Mercy Service Standards.
