Qualifications

• < 140 characters: Experience providing administrative support in case management, discharge planning, and patient transitions.

Responsibilities

• Provide non-licensed clerical support to RN and SW Case Managers.
• Assist with discharge planning and patient care transitions (SNF, home health, etc.).
• Assist locating facilities for placement and arranging transfers.
• Support regulatory compliance and scheduling post-discharge physician appointments.
