Advocate Health is seeking an organized discharge coordinator in the Case Management department at Atrium Health Cleveland. You will assist with discharge planning, prepare packets, and coordinate resources to meet patient needs.

You will interact with ED staff and outside agencies to ensure timely, accurate discharge processes. The role requires strong clerical skills, familiarity with Medicare forms, and the ability to manage multiple tasks.

Formación

• High school diploma or equivalent required or preferred.
• Associate degree in Secretarial Science or health-related field preferred.
• 2 years of office experience preferred.

Responsabilidades

• Assist with discharge planning and packet preparation.
• Coordinate transportation and clothing closet resources.
• Assist with guardianship applications and ED support.
• Support department operational needs and QA audits.
• Complete required Medicare forms within specified timelines.
• Handle reception duties and triage inquiries.
