Job summary

CHI Mercy Health seeks a Care Coordination Assistant to serve as a central hub for patient care from newborn to geriatric within a designated time frame. You will act as the primary receptionist and secretary while supporting safe, therapeutic care and coordinating unit activities with the care team.

You will manage information flow, assist clinicians in fast-paced settings, and ensure timely post-acute referrals and follow-up appointments to support readmission prevention and patient-centered

Qualifications

• High School Diploma/GED required with 3 years in health care or care coordination.
• Preferred degrees include Associates or Bachelor's in related fields; clinical certifications welcomed.

Responsibilities

• Collaborates with and supports the Care Coordination team.
• Manages timely post-acute care referrals.
• Ensures post-acute follow-up appointments to prevent readmissions.
• Delivers patient notifications and collects signatures on documents as required.
• Manages transportation referrals.
