Job summary

Baptist Health in Jacksonville is seeking a Care Coordination Assistant for the Baptist Clay campus. This full-time days position combines clerical duties with bedside support as you move around the hospital delivering Medicare documents and assisting RN Case Managers and Social Workers.

Ideal candidates have 1–2 years of hospital experience, a High School Diploma, and Basic Life Support (BLS). CMA/CCMA or other medical assistant credentials are preferred; Driver's license is required for this

Qualifications

• High School Diploma required.
• Basic Life Support (BLS) required.
• Certified Medical Assistant (CMA) Preferred or CCMA/RMA/NCMA preferred.

Responsibilities

• Provides clinical and clerical support to the Care Coordination Team.
• Coordinates care across inpatient/outpatient/post-acute settings and community resources.
• Assists RN Case Managers/Social Workers with needs and tasks.

Education

Basic Life Support (BLS)

Certified Medical Assistant (CMA) Preferred

Certified Clinical Medical Assistant (CCMA) Preferred

Registered Medical Assistant (RMA) Preferred
