Job summary

Baptist Health in Jacksonville is seeking a Care Coordination Assistant for the Baptist Clay campus. This full-time days position combines clerical duties with bedside support as you move around the hospital delivering Medicare documents and assisting RN Case Managers and Social Workers.

Ideal candidates have 1–2 years of hospital experience, a High School Diploma, and Basic Life Support (BLS). CMA/CCMA or other medical assistant credentials are preferred; Driver's license is required for this

Qualifications

• Certified Medical Assistant (CMA) Preferred or CCMA/RMA/NCMA preferred.

Responsibilities

• Provides clinical and clerical support to the Care Coordination Team.
• Coordinates care across inpatient/outpatient/post-acute settings and community resources.
• Assists RN Case Managers/Social Workers with needs and tasks.

Baptist Health is looking to add a Care Coordination Assistant in our Care Management department at Baptist Medical Center Clay campus.

This is a Full Time Days opportunity.

The role of the Care Coordination Assistant will be mostly clerical, but they will also be moving around the hospital to deliver a Medicare document to obtain a signature and assisting the RN Case Managers/Social Workers with any needs they might have.

Care Coordination Assistant, Job Responsibilities

• Under the direction of a licensed professional, provides clinical and clerical support to the Care Coordination Team.
• Coordinate care across the continuum (inpatient/outpatient/post-acute and community) to assure appropriate utilization of clinical and community resources.
• Duties includes providing patient-centered care for coordinating care transitions of patients and families.

Experience Requirements

• 1-2 years Hospital Experience Required
