Job summary

Baptist Health is hiring a Care Coordination Assistant for the Care Management department at Baptist Clay campus. The role involves clerical tasks and moving around the hospital to assist RN Case Managers and Social Workers with care coordination.

Based at Clay campus, you will coordinate patient care across the continuum and support transitions while ensuring appropriate utilization of resources and Medicare documentation as needed.

Qualifications

• High School Diploma required.
• Basic Life Support (BLS) required.
• Certified Clinical Medical Assistant (CCMA) preferred or CMA, RMA, NCMA preferred.
• Driver's License required.

Responsibilities

• Provide clinical and clerical support to the Care Coordination Team under supervision.
• Coordinate care across inpatient, outpatient, post-acute and community settings to optimize resources.
• Deliver Medicare documents and obtain signatures as needed.
• Assist RN Case Managers and Social Workers with care coordination needs.
