Job summary

TCH Medical Center is seeking a Care Coordinator Assistant to provide non-clinical administrative support for care coordination, discharge planning, and utilization management. The role focuses on timely communications with internal and external customers and maintaining data integrity.

Ideal candidates will have a HS diploma or GED, some college coursework, and at least two years of experience in hospital care coordination or a related field.

Qualifications

• HS Diploma or GED required.
• Some college coursework preferred.
• 2 years' experience in hospital care coordination/case management, home health, quality improvement or a related field required.

Responsibilities

• Provides department-related and other appropriate communications to internal and external customers.
• Assists utilization management processes to maintain compliance with federal, state and managed care guidelines.
• Assists transition/discharge planning and care coordination processes to transition plans and enhance patient/family satisfaction.
• Maintains data integrity.
• Provides professional, efficient and courteous service to the customer while handling confidential information.
• Works with peers, nursing and medical staff and maintains a positive attitude; responds to requests promptly and tactfully.
• Obtains information for responses and communicates pertinent information to staff, customers, and providers; relays messages efficiently.
• Promotes a positive work environment and contributes to the team effort.
