Job summary

Logan Health Medical Center in Kalispell, MT seeks a Care Management Assistant to provide administrative and logistical support to the Case Management team across units including Acute Care, Surgical, Progressive Care, and ICU.

The role focuses on discharge planning, coordinating follow-up care, arranging DME and transportation, and assisting with CMS forms while maintaining confidentiality and strong communication with patients and providers.

Qualifications

• Related coursework beyond high school or experience in a complex administrative support position required.
• Minimum of one (1) year experience in a customer service-focused position required.
• Healthcare work experience preferred.
• Possess knowledge and understanding of medical terminology and medical coding preferred.
• Excellent organizational skills, detail-oriented, a self-starter, possess critical thinking skills and be able to set priorities and function as part of a team as well as independently.
• Commitment to working in a team environment and maintaining confidentiality as needed.
• Excellent verbal and written communication skills including the ability to communicate effectively with various audiences.
• Excellent interpersonal skills with the ability to manage sensitive and confidential situations with tact, professionalism, and diplomacy.
• Possess and maintain computer skills to include working knowledge of Microsoft Office Suite and ability to learn other software as needed.

Responsibilities

• Schedules and coordinates requested follow-up appointments for patients being discharged based on direction from the care management team.
• Orders and coordinates delivery of durable medical equipment as requested by the care management team.
• Assists patients with identified barriers to discharge follow-up, including transportation, appointment location, appointment availability, and scheduling.
• Escalates concerns or barriers to the appropriate care team member.
• Assists in identifying patients who may need follow-up care by reviewing medical records and communicating with care management team members, providers, and nursing staff.
• Assists with arranging resources and services identified by the care management team to support timely and appropriate discharge.
• Delivers CMS forms, including applicable Medicare notices, to patients in a safe, timely, and effective manner and communicates questions or concerns to the appropriate care team member.
• Communicates with healthcare providers, payers, physicians, and community-based services as directed by the care management team to assist with coordinating identified patient needs and services.
• Completes timely and accurate documentation in the medical record in accordance with departmental documentation standards to facilitate communication with care team members.
• Ensures documentation is completed in compliance with applicable guidelines and billing and reimbursement standards.
• Not all of the duties may be assigned to a position.
• Maintains regular and consistent attendance as scheduled by department leadership.
