Job summary

Logan Health in Kalispell, MT seeks a Care Management Assistant to provide administrative and logistical support to the Case Management team across units including Acute Care, Surgical, Progressive Care, and ICU.

You will assist with discharge planning, arrange DME, coordinate transportation, complete CMS forms, and help identify barriers to timely transitions of care while maintaining patient confidentiality and precise documentation.

Qualifications

• Minimum of one year in a customer service-focused position.
• Healthcare work experience preferred.
• Knowledge of medical terminology and medical coding preferred.

Responsibilities

• Schedules and coordinates follow-up appointments for discharged patients.
• Orders and coordinates delivery of durable medical equipment (DME).
• Assists with arranging transportation and identifying barriers to discharge.
• Documents in the medical record per departmental standards and ensures compliance.
