Job summary

Logan Health Whitefish is seeking a Care Manager to coordinate discharge planning and care across the continuum. This 10-hour day shift role supports patients and families and collaborates with interdisciplinary teams to improve outcomes.

Qualifications include Montana RN license (or compact), 2+ years in acute or skilled nursing, experience with discharge planning and utilization review. Logan Health offers benefits, 401(k), tuition reimbursement, and a supportive Planetree-aligned culture.

Qualifications

• Current Montana RN license or multi-state license to practice in Montana.
• Minimum 2 years’ experience in acute or skilled nursing.
• Experience with discharge planning and care management processes.
• RQI Provider (BLS) within 7 days of hire/transfer.
• ACLS within 6 months of hire/transfer.
• PALS within 6 months of hire/transfer.

Responsibilities

• Assess patient needs, plan interventions, and evaluate outcomes.
• Coordinate care across the continuum of care with the team.
• Develop networks of services required by patients receiving care management.
• Track patient progress and maintain confidentiality.
• Participate in quality improvement activities related to patient care.
