Job summary

Under clinical leadership, you will participate in bedside assessments and coordinate post-acute resources in line with the care plan to ensure patients receive high-quality, value-driven post-acute care.

You will assist with discharge readiness, communicate the post-acute plan to patients and caregivers, lead referrals, and provide resources as directed while adhering to regulatory

Qualifications

• Educational Requirements: High School Diploma or Equivalent.
• Associates or Medical Assistant degree preferred.

Responsibilities

• Lead referral processes and provide resources as directed by Care Manager.
• Assist with discharge readiness and preparation for post-acute care.
