Qualifications

• Minimum of 3 years clinical experience required; discharge planning and utilization knowledge preferred.
• Currently licensed as an RN in the State of Florida.
• Case Management certification in ACM, CCM, NACCM, or CMCN preferred.
• Strong communication, interpersonal, analytical, and multi-tasking skills.

Responsibilities

• Facilitate clinical/financial resource management within the care team.
• Plan and coordinate discharge and transitions across the care continuum.
• Link patients to community resources and supports to maximize function.
