Job summary

Qualifications

• Associates Degree in Nursing (RN track) completed.
• Minimum of 3 years clinical experience required.
• Florida RN license currently active.
• Case Management certification preferred (ACM/CCM/NACCM/CMCN).
• Strong communication, analytical and organizational skills.

Responsibilities

• Apply utilization and quality management principles in patient care.
• Lead discharge planning and coordinate resources for safe transitions.
