Job summary

Providence is seeking a Patient Health Advocate to join the Care Management team in Los Angeles. You will work with the Case Manager to collect patient information, monitor care, and help schedule preventive and follow-up visits while assisting with goal setting and ongoing care management.

The role supports a patient-focused culture, providing guidance within a large network of Providence facilities and affiliated practices.

Qualifications

• Coursework/Training: Completed coursework in Medical Terminology or working knowledge of Medical Terminology.
• Within 30 days of hire: National Provider BLS - American Heart Association.
• 5 years' experience in a health care or managed care system.
• 1 year demonstrates knowledge and use of ICD-10 coding.

Responsibilities

• Assist with the daily management of a patient population by collecting detailed information about the patient.
• Monitor care, schedule preventative and follow-up visits, and assist with goal setting and care management.
