Job summary

Providence is seeking a Care Coordinator to support coordinated care delivery across the patient journey. You will liaise with patients, primary care providers, specialists, and hospital teams to remove barriers and ensure timely access to services.

You will advocate for patients, manage communication, and operate under physician direction to deliver compassionate, patient-focused care in a fast-paced environment.

Qualifications

• BLS certification from American Heart Association required within 30 days of hire.
• 1 year of experience in a healthcare setting preferred.
• Associate's Degree is preferred for this role.

Responsibilities

• Coordinate seamless care delivery among patients, PCPs, specialists, hospital or urgent care settings.
• Advocate for patient access to health care throughout their care journey.
• Use communication and time management skills to ensure smooth service delivery.
• Work under the direction of a physician or designee.

Job description

Working under the general direction of a physician or their designee, utilizing communication and time management skills to coordinate seamless care delivery between patients, their PCP, Specialist, Hospital or Urgent care setting. Will act as a patient advocate to assure the patient's access to health care during their entry into and through the service of care.

Required Qualifications:
