Description

The Executive Office of Aging & Independence (AGE) is seeking a mission-driven and dynamic individual to fill our Care Continuum Project Coordinator position. The Care Continuum Project Coordinator is a member of the Executive Office of Aging & Independence (AGE) Care Continuum Team. The AGE Care Continuum is made up of several core programs including the State Home Care Program, Frail Elder Waiver, Clinical Assessment & Eligibility, Community Transition Liaison Program, Money Follows the Person Demonstration, Behavioral Health Older Adult Outreach, Consumer Directed Care, Home Care Provider contracting and monitoring, Electronic Visit Verification for Home Care Providers, and Home Care workforce initiatives. This position is responsible for adaptability and engaging in a variety of cross Care Continuum programs, projects, and initiatives, including; programmatic activities, responding to providers, developing program guidance and conducting training, data aggregation and analysis, assisting in policy development and implementation, and working in a collaborative setting across Health and Human Services agencies.

In this highly adaptive, multifaceted role, the Care Continuum Project Coordinator works across all team programs, supports team members, and leads projects in a nimble, collaborative manner. Through program connectedness, the incumbent helps bring ideas to execution, approaching. projects at different stages—sometimes helping to develop policy and other times analyzing data and consumer profiles to support decision-making and reporting. In this role, the Care Continuum Project Coordinator works with AGE staff and across Executive Office of Health and Human Services (EOHHS) agencies and with other agencies including but not limited to MassHealth Office of Long-Term Services & Supports, The Department of Mental Health, Mass Ability, and the Department of Developmental Services to ensure programmatic requirements are met for consumers served through the AGE Care Continuum programs.

Reporting to the Director of Home & Community Programs, the incumbent supports critical aspects of the agency's mission by ensuring adherence to AGE Care Continuum requirements, programs are implemented according to regulation, sub-regulatory guidance, and policy across the Commonwealth, providing services, quality measurements are in compliance, and reporting meets expectations as outlined. This successful candidate must possess project and program management experience and be able to communicate clearly conveying and articulating information in writing and orally to AGE managers, senior staff, and providers. The position will be required to travel across the Commonwealth 20% of the time.

Duties and Responsibilities:(These duties are a general summary and not all-inclusive)

  • Support the implementation, coordination, monitoring, quality improvement and development of initiatives related to the operation, optimization, and overall management of AGE Care Continuum programs and initiatives.
  • Support and collaborate with the AGE Care Continuum team members in their role.
  • Assist other Executive Office of Health and Human Services (EOHHS) agencies with technical assistance related to the operation and management of the AGE Care Continuum programs and initiatives.
  • Track, identify and complete actions adhering to programmatic management deadlines for identified AGE Care Continuum programs and initiatives.
  • Identify, recommend, draft procedures, sub regulatory guidance, and policies for identified AGE Care Continuum programs and initiatives for oversight and administration as well as contract guidance.
  • Assist in development of and implement standards to monitor and evaluate AGE Care Continuum programs and initiatives including activities ensuring equity across the Commonwealth of the quality of services being provided to participants.
  • Support data collection, reporting, tracking, trend identification, compliance monitoring and remediation.
  • Assist in response to time sensitive requests for information, and public document requests.
  • Support the team in Program Health & Integrity initiatives including report and record review, identification of trends, recommending next steps, and assisting in course correction and technical assistance.
  • Assist with scheduling for Assistant Secretary and team related meetings and activities.
  • Support cross integration management of consumer records within AGE cloud-based database.
  • Initiate action with ASAPs to ensure programmatic compliance related to regulations, sub regulatory guidance, and program instruction.
  • Review AGE Care Continuum program and initiatives reporting including performance measures, aggregating data, preparation of data reports, and presentation of findings.
  • Assist in identifying, developing, and conducting education and training of AGE Care Continuum programs and initiatives program requirements and changes.
  • Advise the Assistant Secretary of Care Continuum, Director of Home & Community Programs, the Clinical Nurse Manager, and the Deputy Director of Care Transitions, and other members of the Care Continuum team on issues related to program and initiatives.
  • Support the team in development of and rendering of training related to program and sub regulatory information, guidance, and requirements.
  • Other duties, as assigned including traveling across the Commonwealth

Required Qualifications:

Preferred Qualifications:

  • Expert knowledge of Aging Services Network, the State Home Care Program, Long Term Care options, Nursing Facility, and Care Transition programs.
  • Functional knowledge of community resources and ability to case conference with AGE staff/ other agency staff and aging network staff on consumers enrolled in AGE Care Continuum programs and initiatives and those enrolled in other EOHHS programming.

Qualifications

MINIMUM ENTRANCE REQUIREMENTS: Applicants must have at least (A) four years of full-time, or equivalent part-time, professional, administrative or managerial experience in business administration, business management or public administration the major duties of which involved program management, program administration, program coordination, program planning and/or program analysis, and (B) of which at least one year must have been in a supervisory capacity, or (C) any equivalent combination of the required experience and the substitutions below.

Substitutions:

I. A Bachelor's degree with a major in business administration, business management or public administration may be substituted for a maximum of two years of the required (A) experience.*

II. A Graduate degree with a major in business administration, business management or public administration may be substituted for a maximum of three years of the required (A) experience.*

III. A Bachelor's or higher degree with a major other than in business administration, business management or public administration may be substituted for a maximum of one year of the required (A) experience.*

*Education toward such a degree will be prorated on the basis of the proportion of the requirements actually completed.

Salary placement is determined by a combination of factors, including the candidate’s years of directly related experience and education, and alignment with our internal compensation structure as set forth by the Human Resources Division’s Hiring Guidelines. For all bargaining unit positions (non-management), compensation is subject to the salary provisions outlined in the applicable collective bargaining agreement and will apply to placement within the appropriate salary range.
