Qualifications

• Master’s degree from an accredited school is required.
• Two years of direct clinical experience with the target population is required; internship may substitute.
• Handle with Care (HWC) Certification required for Behavioral Health Unit.

Responsibilities

• Coordinate care across inpatient, emergency, and ambulatory settings to ensure integrated delivery.
• Engage patients and families to collaborate, advocate and problem-solve and to support discharge planning.
• Monitor progress and adjust discharge plans based on patient condition and family needs.
• Educate patients and families about evidence-based self-management strategies and healthcare navigation.
• Facilitate psychosocial support and education, and promote safety and compliance with policies and standards.
