Job summary

Total Care Connections in Tucson is seeking a Care and Scheduling Coordinator to ensure safe, compassionate care and seamless scheduling for clients and caregivers. This on-site role supports families, clients, and the care team with a trusted, problem-solving partnership.

You will conduct in-home assessments, develop individualized care plans, and coordinate starts of care while collaborating with Recruiting, HR, and Operations to keep the team aligned and focused on quality care.

What You'll Do

As a Care Coordinator, you'll help ensure every client receives safe, consistent, and compassionate care while supporting our caregivers to succeed.

• Conduct in-home client assessments and supervisory visits, ensuring care is being delivered safely and according to the client's needs.
• Develop, implement, and regularly update individualized care plans.
• Coordinate and schedule starts of care and maintain ongoing staffing schedules, handling any challenges that may arise, such as call-outs.
• Build genuine, trusting relationships with clients, families, caregivers, and referral partners.
• Listen to clients and families, understand their concerns, and take ownership of finding solutions.
• Support caregiver onboarding, training, and ongoing supervision to ensure caregivers are equipped to provide exceptional care.
• Monitor the quality and consistency of care and proactively address concerns before they become larger issues.
• Ensure compliance with company policies, Arizona regulations, and medical insurance requirements.
• Maintain accurate, timely, and thorough documentation.
• Partner closely with Recruiting, HR, and Operations to ensure the entire care team is aligned and supported.
• Identify opportunities to improve the client and caregiver experience and contribute to continuous quality improvement.
• Represent TCC with professionalism, compassion, and integrity in every interaction.

What We're Looking For

• At least 1 year of experience in care coordination, care management, care supervision, scheduling or a related role.
• Previous experience in home care, caregiving, or community-based healthcare.
• A genuine passion for helping people and improving the lives of clients and families.
• Strong communication, relationship-building, and customer service skills.
• Excellent organization and attention to detail.
• The ability to manage multiple priorities while remaining calm, responsive, and solutions-focused.
• Reliable transportation and a clean driving record.
