We’re hiring a Care Advocate to serve as the trusted point of contact for a select group of member families. This role is equal parts relationship manager, project manager, care coordinator, and strategic partner. You’ll guide families with warmth and clarity, orchestrate support across our provider network, and ensure every detail is beautifully handled. You’ll also act as the central communicator between Appian, our members, and local care partners on the ground.

What You’ll Do
Guide Families with Calm Confidence
- Serve as the dedicated, long-term advisor to a select group of member families
- Conduct holistic assessments and translate insights into personalized Roadmaps
- Triage needs across medical, residential, legal, financial, and lifestyle domains
- Maintain ongoing communication, adapting plans as needs evolve
Manage Care Like a Project
- Coordinate services across a team of local partners and care professionals such as GCMs, nurses, attorneys, and organizers
- Track tasks, milestones, and deliverables with precision
- Keep meticulous records in our CRM and ensure seamless documentation
- Anticipate issues before they arise and make families feel cared for, not managed
Oversee External Partners and In-Home Support
- Build and maintain strong relationships with geriatric care managers, home care providers, and other specialists
- Assign and oversee GCMs, ensuring accountability, communication, and brand alignment
- Handle scheduling, feedback loops, and case-specific delegation to local partners
- Ensure our in-home partners deliver a member experience that reflects Appian’s standards
Support Internal Growth and Systems
- Offer thoughtful feedback to refine internal workflows and member experience
- Collaborate with leadership to scale systems, train future hires, and shape the role over time
What Makes You a Great Fit
- Experience in aging services, healthcare, care navigation, or senior support (Examples: social work, nursing, OT/PT, care management, health coaching, or personal or family experience)
- You build trust quickly and know how to hold space for families navigating big decisions
- You think in checklists, timelines, and priorities and nothing falls through the cracks
- You’re emotionally attuned, clear under pressure, and comfortable in ambiguity
- You’re a natural communicator who brings warmth and steadiness to every interaction
- You take initiative, solve problems, and treat the work like it’s your own
- You’re tech-comfortable and familiar with tools like Google Workspace and CRM systems
- You’re open to occasional travel such as team fly-ins, in-person meetings, or family touchpoints
