Job summary

Neighborcare Health is seeking an Eligibility Specialist to help patients enroll uninsured individuals into insurance coverage and community programs, reducing financial barriers to care.

You will verify household size and income, determine eligibility for financial assistance, and place patients on the sliding fee discount scale where appropriate, partnering with care teams to promote equitable access to healthcare.

Qualifications

• Strong communication skills with patients and staff; active listening and professional interaction.
• Proficiency with computers and electronic health records.
• Fluent in English with basic math for income calculations.

Responsibilities

• Conduct in-depth interviews to assess eligibility for insurance programs and sliding fee discounts.
• Explain program rules clearly and support patient understanding.
• Gather and document eligibility information including residency, citizenship, household size, and income.
• Assist patients in obtaining documentation and completing applications.
• Determine initial and renewal eligibility for publicly sponsored programs per policy.
• Perform data entry and research eligibility guidelines to answer questions.
• Stay updated on health insurance programs and eligibility requirements.
• Communicate coverage options to patients clearly and accessibly.
• Collaborate with Front Desk, Patient Contract Center, and Insurance Coordination to connect patients to eligibility services.
• Support insured patients with renewals and plan changes to maintain coverage.
