Job summary

Mercy is seeking a Nurse Navigator for CV Surgery at Mercy South to guide cardiovascular surgery patients through the entire care journey. You will be the central point of contact, coordinating pre- and postoperative education, assessments, and timely scheduling.

You will collaborate with surgeons, cardiologists and interdisciplinary teams to streamline transitions of care, reduce readmissions, and support delivery of high-quality outcomes.

Responsibilities

• Coordinate CV surgery patient through the full care journey
• Provide comprehensive pre- and post-operative education
• Conduct assessments, and ensure timely scheduling, testing and follow-up
• Collaborate with surgeons, cardiologists and interdisciplinary teams
• Support transitions of care to reduce readmissions and improve outcomes
