Responsibilities

• Assessment/Diagnosis – Performs initial, ongoing, and functional health status assessment.
• Outcomes Identification/Planning – Documents planned interventions to achieve outcomes.
• Implementation/Evaluation – Evaluates and revises the care plan based on evaluative data.
• Leadership – Actively participates in process improvement for quality and safety.
• Operations – Supports safety, quality, and efficiency as part of a multidisciplinary team.
