Qualifications

• High School Diploma or GED required.
• 6 months of experience in the healthcare setting within the last year; 6 months in acute care preferred.

Responsibilities

• Provides and collaborates direct patient care under the direct supervisor of a licensed caregiver.
• Responsibilities may include: personal care, activities of daily living, vital signs, intake and output, ambulation, interventions, and portions of the patient care plan.
• Addresses and/or directs questions or concerns from patients, families, caregivers, or health care team members.
• Maintains accurate medical record documentation related to the care delivered.
