Job summary

Community Health Network’s Cardiology Clinic in Indianapolis Northside is seeking an Ambulatory Triage Registered Nurse to perform phone triage, patient education, scheduling, and care coordination for cardiology patients. The role combines onsite clinic work with remote work from home after training completes.

The RN must be licensed in Indiana, with 2+ years of nursing experience; cardiology or medical office experience is preferred, along with strong teamwork and compassionate patient care.

Qualifications

• Graduate of NLN, CCNE, ACEN, NLN CNEA accredited nursing school, or three years of related nursing experience.
• Licensed RN with Indiana license listed in NLC.
• Two years or more of nursing experience.
• Cardiology experience preferred.
• Previous medical office/outpatient experience preferred.
• Excellent team skills.
• Compassionate and caring.
• Accountable and self-directed.

Responsibilities

• Phone triage of patients and documenting interactions.
• Provide patient education and explain care plans.
• Schedule appointments and coordinate follow-up when needed.
• Contact patients with results and coordinate care across teams.
• Handle prescription refills and general care coordination as required.
