Job summary

Community Health Network is seeking an Ambulatory Registered Nurse to support primary and specialty care clinics. The RN will triage calls, educate patients, schedule appointments, handle medication refills, referrals, and care coordination.

They will float to offices in three regions: north side of Indianapolis, Anderson, and Kokomo, with EPIC experience a plus. Qualified candidates must be a licensed RN in Indiana with 2+ years of nursing experience; outpatient office experience is preferred.

Qualifications

• Nurse must be a licensed RN in Indiana with active license.
• Two years or more of nursing experience.
• Experience in outpatient/medical office settings preferred.
• EPIC electronic medical record experience is a plus.

Responsibilities

• Triaging calls and patient education.
• Scheduling appointments and medication refills.
• Performing referrals and care coordination.
• Rooming patients and supporting care across primary and specialty clinics within three regions.
