Job summary

Community Health Network is seeking an Ambulatory Triage Registered Nurse to join the Noblesville Internal Medicine Care team in Noblesville, Indiana. The role includes phone triage, patient education, scheduling, prior authorizations, prescriptions, referrals, and care coordination onsite.

The ideal candidate will have RN licensure in Indiana, strong communication and teamwork skills, and experience in a medical office setting. EPIC experience is a plus.

Qualifications

• RN licensure in the state of Indiana.
• Graduation from an accredited nursing program (NLN/CCNE/ACEN/NLN CNEA or equivalent).
• Two years or more of nursing experience required.

Responsibilities

• Perform phone triage and patient education.
• Schedule appointments and manage referrals.
• Handle prior authorizations and prescription refills as needed.
• Coordinate patient care across teams and services.
• Assist with other related tasks in ambulatory care settings.
