Qualifications

• Three+ years in healthcare settings, independent work.
• Experience interacting with patients, physicians, and staff.
• Ability to work autonomously in clinical settings.
• Strong communication and organization in fast-paced environments.
• Experience with EHRs and enrollment/care-management tools.
• Certifications such as CMA/CHW/CPhT/RHIT or equivalent.

Responsibilities

• Maintain daily on-site presence at partner cardiology practices.
• Pre-screen patients for eligibility and enroll into the care program.
• Educate patients about services and coordinate enrollment.
• Document patient information in care-management systems.
• Coordinate with practices to update care journeys and transfers.
• Deliver educational presentations to physicians and staff.
• Handle inbound enrollment inquiries and support special projects.
