Job summary

Qualifications

• Current Washington State RN license.
• Bachelor’s degree in Nursing; ADN with equivalent experience may be considered.
• 3–5 years of recent clinical experience in cardiology or related specialty.
• Strong clinical judgment, communication, and care coordination skills.
• Knowledge of clinical workflows, payer guidelines, and case management.

Responsibilities

• Screen patients to identify navigation candidates and manage an assigned caseload.
• Coordinate referrals and connect patients with clinical, financial, and community resources.
• Assess barriers that may affect the plan of care and address them.
• Coordinate referrals to nutrition, social work, home health, hospice, and community resources.
• Track interventions and outcomes; collaborate with the healthcare team.
• Monitor treatment plans and notify providers when reassessment is needed.
• Contribute to quality improvement and program development.
