Qualifications

Responsibilities

Coordinate care and educational support to patients and families.
Collaborate with multidisciplinary team to develop and implement up-to-date plan of care.
Refer patients to appropriate specialists using clinical protocols and provide support.
Lead committees and develop evidence-based protocols to improve patient experience.
Coordinate multidisciplinary efforts and utilize resources efficiently and effectively.
Assist in education, screening, prevention, and treatment initiatives for the community.
