Responsibilities

• Assess needs for new patients referred to a specialist prior to their first visit.
• Document patient assessments and interventions in the EMR.
• Refer new patients to the program’s supportive services as needed.
• Provide educational and emotional support throughout intake and treatment planning.
• Coordinate diagnostic studies prior to first visit to expedite treatment start.
• Develop relationships with referring physicians and communicate follow-up plans.
• Complete intake logs and report monthly for outcome metrics.
• Participate in quality improvement initiatives.
